Hand pain is a common complaint, but not all discomfort stems from carpal tunnel syndrome. While carpal tunnel can cause numbness, tingling, and pain in the wrist and fingers, several other conditions could be to blame. Understanding the true cause of your hand pain is essential for finding the right treatment.

In this article, we’ll explore the most common causes of hand pain beyond carpal tunnel and discuss how to identify them.

Trigger Finger: When Your Finger Gets Stuck

Trigger finger (stenosing tenosynovitis) occurs when a finger’s tendon becomes thickened and irritated, making it difficult to straighten.

Common Symptoms

  • Finger stiffness, especially in the morning
  • A popping or clicking sensation when moving the finger
  • Pain and tenderness at the base of the affected finger
  • Finger locking in a bent position

This condition often develops in people who perform repetitive gripping motions.

Treatment Options:

  • Rest and splinting
  • Anti-inflammatory medications
  • Corticosteroid injections
  • In advanced cases, a minor surgical procedure to release the affected tendon

De Quervain’s Tendinosis: Thumb and Wrist Pain

If you’re feeling pain on the thumb side of your wrist, you may have De Quervain’s tendinosis. This condition is caused by irritation and inflammation of the tendons at the base of the thumb.

Common Symptoms:

  • Pain and swelling near the base of the thumb
  • Difficulty gripping objects
  • Pain that worsens with wrist movement

This condition is often seen in new mothers, repetitive hand workers, and individuals who frequently use smartphones.

Treatment Options:

  • Immobilization with a splint
  • Anti-inflammatory medications
  • Physical therapy
  • Injections or surgery in severe cases

Extensor Carpi Ulnaris Tendinitis: Pain in the Outer Wrist

A less common cause of hand pain is extensor carpi ulnaris tendinitis, which affects the ulnar dorsal wrist (the outer side of the wrist near the pinky finger). This condition occurs due to overuse or repetitive stress, often in tennis players, weightlifters, and office workers.

Symptoms Include:

  • Pain along the ulnar side of the wrist
  • Weakness when gripping objects
  • Swelling and tenderness

    Treatment Options:

    • Rest and avoiding repetitive activities
    • Wrist braces or splints
    • Anti-inflammatory medications
    • Physical therapy

Dupuytren’s Contracture: A Genetic Hand Condition

Dupuytren’s contracture is a genetic condition that causes thickened tissue (nodules) to form in the palm, leading to finger stiffness and curling over time. It is most common in people of European or Japanese descent and can be passed down through families.

    Key Signs to Watch For:

    • Small, firm nodules in the palm
    • Fingers gradually bending inward
    • Difficulty straightening the affected fingers

Although Dupuytren’s is not painful at first, it can severely limit hand function if left untreated.

    Treatment Options:

    • Stretching and hand therapy
    • Corticosteroid injections
    • Enzyme injections to break down thickened tissue
    • Surgery in advanced cases

Arthritis at the Base of the Thumb

One of the most common causes of hand pain, particularly in women over age 50, is arthritis at the base of the thumb (thumb CMC arthritis).

    Symptoms Include:

    • Pain and swelling at the base of the thumb
    • Difficulty gripping or pinching objects
    • Joint stiffness and reduced mobility

Interestingly, over 50% of people with thumb arthritis also have carpal tunnel syndrome. Treating both conditions together can provide significant relief.

    Treatment Options:

    • Bracing and anti-inflammatory medications
    • Cortisone injections
    • Physical therapy
    • In severe cases, surgical procedures like joint fusion or joint replacement

If you’re dealing with persistent hand pain, stiffness, or weakness, it’s important to get an accurate diagnosis. Many conditions have overlapping symptoms, and proper treatment depends on identifying the underlying cause.
